Management Of Post Septic Sequale Of Hip With Neck Of Femur Non-Union In Children

BACKGROUND: Coxa vara and Non-union of the femoral neck after septic hip arthritis is a very rare disease. The aim of this study was to present a case with non-union of the femoral neck, with coxa vara after neonatal septic arthritis of the right hip, and treatment of the non-union with proximal femoral valgus osteotomy.

METHODOLOGY: Two paediatric cases of post septic sequale of hip non-union neck of femur with coxa vara presenting at Orthopaedics department, Institute of Child Health, Egmore were selected for this study

RESULTS: Two cases who had suffered neonatal septic arthritis and painful right hip now demonstrated painless limping to the hip, pelvic obliquity, Trendelenburg-Duchenne gait, limitation of hip abduction, lower-extremity length discrepancy with shortening of femur, and walking on their tiptoes. In addition, a viable femoral head, coxa vara, and pseudo arthrosis of the femoral neck were observed. Valgus osteotomy of the proximal femur and bone grafting with fibular strut graft were performed. Follow up were done at 2nd, 6th, 10th months. At the last follow-up postoperative evaluation, these patients showed successful consolidation of non-union, correction of Trendelenburg gait, restored lower extremity alignment.

CONCLUSIONS: In the case of coxa vara and non-union of the femoral neck, proximal femoral valgus osteotomy and bone grafting with fibular strut graft of the non-union resulted in ossification of the femoral neck as the epiphyseal plate was placed at right angle to the compressive forces.
